"Dean's actually alive?" Listen, he wanted to be supportive of Castiel, and he knew that Lucifer said the man was hard to kill, but Istredd did think this entire time there was no way he did survive. He was pulled underwater after being speared by a sea creature, apparently. Maybe an angel would have survived that, but a human? So he looks genuinely shocked by that.
"I know you are worried, but Castiel promised me that he wouldn't let Dean hurt me to get to you." Istredd is very much not in the know right now, so his instinct is that Lucifer was afraid of the same thing as before, with the Mark. Dean knowing Istredd's identity was a concern for him, fine, but Castiel was very sincere about his promise.
